Isle of Man to consider water fluoridation in childhood oral health plan

 

The Isle of Man will look into the benefits of water fluoridation as part of its integrated plan to tackle the issue of children’s oral health on the island.

Published on 20 September, Amalgamated Implementation Plan for Dental Strategy and Oral Health Strategy incorporates strategies such as increased access to dentists and the introduction of a supervised toothbrushing scheme. It will be laid before the Tynwald at their October sitting.

The plan also includes further research into the benefits of water fluoridation. In June, Tynwald members voted in favour of producing a research paper into water fluoridation. This will now be considered by the Council of Ministers by the end of 2023.

The strategies were based on recommendations made by The Social Affairs Policy Review committee in January. It was determined that action was needed after investigation revealed 3,014 Manx people were waiting for NHS dentistry. Of those waiting for access to the Isle of Man’s 12 dental practices, 780 were children.

The committee described hospital admissions of Manx children for tooth extractions as ‘remarkably high’, emphasising that this was ultimately ‘preventable’.

Reducing tooth decay

Water fluoridation was put forward as a cost-effective alternative to fluoride varnish by Isle of Man Health Minister Lawrie Hooper. However, twice-annual varnish treatments for children under 11 have thus far been included as part of the integrated plan.

The benefits of water fluoridation have been acknowledged for some time, with local authorities consulting on its introduction since 2013.

In 2022, Nigel Carter, chief executive of the Oral Health Foundation, said: ‘We believe that water fluoridation is the single most effective public health measure there is for reducing oral health inequalities and tooth decay rates, especially amongst children.

‘We welcome these proposals and believe they represent an opportunity to take a big step forward in not only improving this generation’s oral health, but those for decades to come.’

BBC - Integrated plan to tackle oral health issues in children

 An integrated plan which aims to improve oral health for children has been published by the Isle of Man's public health directorate.

It brings together two existing government strategies and incorporates recommendations approved by Tynwald.

Among those are greater access to dentists for those aged under 11 and the reintroduction of a supervised toothbrushing scheme.

Dental therapist Clare Burton appointed to BFS Council

The British Fluoridation Society (BFS) has appointed dental therapist Clare Burton to its Council. 

Clare (pictured) works for the Community Dental Service in Norfolk and Waveney and is also an active committee member of the British Association of Dental Therapists (BADT).

She has been a keen advocate for water fluoridation throughout her career and has appeared on the BBC current affairs programme 'Question Time' in relation to oral health and equality.

Clare said: 'My support for water fluoridation chimes perfectly with my clinical work with children, which includes safeguarding, as well as being a mother of two young children.

Fluoride to be added to Nottingham's tap water to tackle rising tooth decay in children

 The council is planning to add fluoride to Nottingham's water supply after statistics revealed more than two-thirds of children in the city have tooth decay.

Figures from 2022 show that 34.2% of five-year-olds in the city had obvious signs of dental decay, compared to 23.7% across the rest of the country.

The average number of teeth affected by decay in Nottingham's five-year-olds is 1.3, while the England average is 0.8.

Council documents say the the "significantly worse" levels of tooth decay in Nottingham's children is a sign of "persistent health inequality experienced by the Nottingham City population, with the proportion of five-year-olds with visible signs of tooth decay remaining similar since 2015/16."

Nottingham City Council is planning to add fluoride to water in the area in a bid to tackle the problem.

How can fluoride improve dental health?

Studies show that fluoride, a naturally occurring mineral, strengthens the outer surface of teeth, making them more resistant to decay.

It is added to dental products including many toothpastes, but can also be put into tap water to reach whole populations.

The World Health Organisation recommends water fluoridation as an "effective and safe public health intervention".

Fluoridated water is currently supplied to 10% of the population in England - including parts of Nottinghamshire such as Mansfield, Bassetlaw, Ashfield and some parts of Newark and Sherwood.

Nottingham City Council says water fluoridation in the city would "complement other Nottingham City oral health initiatives".

The council is working with schools, early years settings and their workforce to improve oral health in young people and vulnerable adults.

NHS England has given Nottingham City and Nottinghamshire County Councils £100,000 to fund the distribution of toothbrushing packs to food banks and other community centres.
